I'd suggest trying proviron with trt and maybe doxazosin 1-2mg.

I used to take cialis daily but I don't recommend that to anyone anymore as you develop tolerance over years and it literally stops doing anything despite research saying otherwise.

I would put the blame on aging/underlying vascular issues if you are or eventually become a poor responder.
Eventually, most that do not do well on PDE5i will need to use intracavernosal injections such as Trimix to overcome this!
In severe cases of ED, where they do not respond to such therapies the next move would most likely be a penile implant.
Has anyone heard of TriMix. It is an injection of alprostadil, a prostaglandin the prostate makes to get the cavernosa to hold blood and get a firm erection. It is dose-related for effect and duration is influenced by adding varying amounts of papaverine and/or phentolamine to it. As it is synthetic of a natural substance, it works in about 10 minutes and can last from 30 minutes to over 2 hours depending on innate vascular ability of the penis and the dose.
Implants are very pricey, about 20 thousand. Insurance will occasionally pay but not often. It's surgery. When it breaks down its replacment of the entire unit- no patch-ups.
I am not a fan but also know many who are happy with them. And, I have seen patients with the fourth one in place, and out of about 80 thousand and the fourth one does not work. It destroys the cavernous tissue so no going back to any other method.
Look before you leap. Try trimix before implant.
