I have been talking to the Men's Clinic in Alabama. They discussed what would happen the first visit with me and one thing that seems standard is an ultrasound of your penis (just the base I think) and scrotum. They said it does not really hurt (not sure why it would). But still a little unsure about this. Anyone got any experience?

Find out how much they want for the test. It is used to see if you have varicocele, calcifications, etc and possibly used to measure testicular size. They may want to do it to justify a HCG prescription. I am actually impressed that they provide that test.

They said the total first visit is $199 (includes blood work, ultrasound, test dose of an erection inducing med--apparently they only give 30% of a dose to see if there is any reaction--, and to figure out a TRT plan). He said something about also using ultrasound to look for blockages.

Is this something to be concerned about, seems odd and maybe a little uncomfortable to do it on the testicles.

From what I have read on here that is good. He said they had oral and injections for the erections. It will be determined which one based on the evaluation and discussion before they give it to me. I'd rather not take something oral that can affect bp and other things and from what I have read the Trimix is AWESOME. What can I do to make sure I get Trimix or whatever else they may want to use similar to it?

It's done by a little hand held device.

Only requires that you pull your penis out and they rub a small sensor over the base of your penis to measure blood flow and volume.

It's no big deal whatsoever.

I had this test done at Cornell Sexual Medicine in NYC. "Duplex Doppler Penile Ultrasound." It appeared on my insurance as "Corpus Cavernosum Study." They injected a drug (alprostadil) at the base of my penis to induce an erection, because it would be unlikely that I could achieve and maintain one in such a setting for adequate testing. They did images of my scrotum to check for varicocele. And they did images of the penile shaft as well. The awkward part was self-stimulation to record dynamic images of blood flow, leakages, and muscle activity (or lack there of) during arousal/climax. These tests were needed to diagnose a biological (cardiovascular/hormonal) basis for sexual dysfunction (rather than psychological). Testosterone, Aromatase Inhibitor, PDE-5 inhibitor, and L-arginine were prescribed after the tests. If paying for it is not a problem, it's the best way to get an accurate idea of what's going on and what will help.

Samuel, that sounds like it was pretty awkward basically having to masturbate while they did the ultrasound. What did they find on the ultrasound for you?

They are trying to access what your flacid blood flow vs your erect blood flow is like. Check for restrictions, venous leakage, and other things. The actual ultrasound doesn't hurt. They will inject you with a vasodialator. From their they can decide what type of ED you are experiencing and the proper course of treatment to go with. They should also give you a receipt with proper codes so you can submit to private insurance. If you have Medicare and the doctor isn't opted out they shouldn't be charging you for the consultation at all.

I am the Director of Clinic Operations for Boston Medical Group, so we use this same procedure in our office every day. The actual ultrasound and consultation takes about 15-30 mins. Then they will test your response from the diagnostic application of medication. Generally, you should get an erection that lasts from 20 mins to 1 hour in the office if they are good at what they do. From there they most likely will prescribe you a injectable to use if you have already tried viagra, levitra, or cialis with no success. It is applied the same way the diagnostic test was. It is virtually painless. They will used a diabetes applicator which makes it very easy to use at home. I am sure they will probably also do some bloodwork for Testosterone as well.

If this is the initial Physical for Trimix with Ultrasound the visit will take an hour.

They will inject with a small dose of what they believe you will respond to and see how you respond.

You will get a nice erection in the office...so be prepared

Once done, they will give you some meds to get your erection down faster so you don't go back to the office with a raging monster in your pants LOL!

Most likely Trimix.

There is Quadmix, but it's not prescribed as often.

Yes, they will want to see your erection and feel it for hardness.

Don't be shy, they see this everyday.

It was admittedly awkward, but the doctor was extremely professional. It's hard to treat a biological sexual disorder without objective measures. This is an objective measure, and knowing that made the test seem appropriate however awkward. If you aren't having any sexual dysfunction, I'm not sure the test I had is really necessary. Maybe just check for varicocele only. Anyway, they found that my blood flow (cardiovascular health) was poor. Low testosterone certainly is a factor in that, but everything is related: Low T -> low energy/drive -> less active/fatigue -> carb/sugar/fat-heavy diet to compensate for low energy -> weight gain/obesity -> elevated lipids and blood pressure -> increased conversion of T to estrogen from aromatase in excess fat -> Low T and back around again - you could add sleep apnea and sexual dysfunction in there as well as both consequences and contributing factors. Ultimately, there is a strong correlation between poor circulation in the penis/ED and heart disease. My doctor said my readings would have been alarming for an 80 year old man let alone a 35 year old one. He said we had to treat the low T and improve cardiovascular tone with diet and exercise. He also found my E2 was at 88 which was masking any benefit of TRT, so he wanted to manage that with an AI. I've been back to see him since, and I show much improvement from the baseline test. So - worth the trouble and awkwardness in my opinion.

I don't know if it's 30% dosage but they do want to see how you respond and you will.

You will get complete erection but it's duration will be shorter then what you'd use at home when having sex.

Believe me, it's not as embarrassing as you'd think...very clinical.

I have just been told by my best friend who is a nurse and did a rotation with urology, that guys who come in with only minor problems getting an erection or keeping one usually walk out with no ED drugs. He said one guy they gave the shot to, a small dose to just see, he ended up getting hard and even harder without the full dose and the doctor told him if he could get aroused on his own that much more after injection then no meds were needed.

He said another guy came in and had some serious trouble, but during the ultrasound, which he said lasted 20 minutes before they wanted to use the injection he got a little aroused with all the handling of his goods so doc sent him home.

They are just telling you 30% so that you have low expectations. If they told you that you would have 100% and you only got 60% you would be disappointing. When you go in thinking you will have 30% and end up with 80% you will be ecstatic.
