I have hyperparathyroidism normocalcemic, what’s next?

Well, it’s confirmed. I have hyperparathyroidism normal calcium variant. TRT is contraindicated for hyperparrathyroidism normocalcemic because testosterone increases urinary calcium loss.

I couldn't tolerate vitamin D on TRT at all. Now it's better tolerated. I’m only taking 600 units of vitamin D = 26 vitamin D level.. This explains why my diabetes gets worse when I take vitamin D.

My symptoms are as follows, bone pain, body aches, muscle pain, jaw pain, headaches, and pain in the testicles and also rapid change in the size of my testicles, being full size one moment and completely retracted the next.

I also get pain, tingling sensations in my throat after taking the vitamin D supplement. It makes a speech nearly impossible at times.

My muscles go from being hard one hour, eating and to being soft in an instant. I also found that playing with my vitamin D supplement dose causes changes in muscle hardness, and how I feel, but never staying in that sweet spot for very long.

I also have changes in the color of my teeth and a strange taste in my mouth.

Hyperparathyroidism, normal calcium variant also causes low testosterone. Throw in previously being overweight and now you developed diabetes.

It’s the perfect storm!

I was able to fill the container with urine yesterday before the 24 hours was up, I ran out of space. I’m so thirsty all the time.

I have to underdose vitamin D to keep the symptoms at bay but at the same time I go from feeling the effects of low vitamin D, increase the dose and then I get these symptoms.

Maybe when I get this situation corrected I’ll be able to handle a bigger dose of vitamin D without issue.

I’m starting to wonder if all my issues on TRT were related to hyperthyroidism. I couldn’t tolerate any vitamin D on TRT once I stopped the beta blocker.

I do not plan on starting TRT again if that’s what you’re wondering. I still feel better off TRT and I my usual upbeat personality even though I feel like crap at times.

Pain is an old friend of mine. I don’t let anything get me down.

And your parathyroid hormone level is increased?
Kidneys are ok?
Other mineral levels and vit D are in range?
I did get a parathyroid hormone test a few months ago and it was low normal but that was after taking 6000 mg of calcium the same day of the test. Taking 600 units of vitamin D my vitamin levels are around 26 at this dose.

Taking the vitamin D supplement triggers me to urinate like crazy and all the symptoms mentioned above ensue. If I don’t take vitamin D, as in I have low vitamin D, I don’t have any of the symptoms only the symptoms of low vitamin D.

All other vitamins and minerals are normal.

Back in 2019 I was taking 2000 units of vitamin D and I didn’t have these symptoms. I did get some slight burning in my skin, which is why I stopped.

It does seem as time has progressed my tolerance to vitamin D has changed.

If you don't take vit D then other problems arise?
Symptoms of low vitamin D. Urination actually slows down and becomes difficult.
What happens if you continue supplementing calcium?
If I take calcium supplement the bone pain and a lot of these symptoms disappear, but eventually I start to get severe abdominal cramping. It’s like I’m a yo-yo. I can never stay in the sweet spot for long.

When I take the vitamin D supplement, my throat is really irritated around where my Adams apple and thyroid are located. It’ll even force me to have coughing spells. It feels like insects are crawling around in my throat.

What's your plan now?
I’ve messaged my endocrinologist. I guess he’ll have a plan.

You know when I first took Jardiance, I felt amazing on the first dose. I read that Jardiance temporarily affects parathyroid hormone when you first start taking it.

I now may have a reason why all the type two diabetes drugs only worked on the first dose only.

Hyperparathyroidism makes it difficult to control diabetes.

The stars are aligning for me now.

I'm not convinced that parathyroid is the root cause.
I mentioned it before, when my body is in ketosis I pee abnormal amounts. I tried adding salt potassium.... eventually just adding a pill of electrolyte complex normalized it. It might be similar to you on carnivore. Low insulin makes it hard to retain minerals.
Well, I guess if you add a electrolyte complex then you will report stomach/gut issues. Maybe what @FunkOdyssey mentioned is a good idea: gastrointestinal inspection.
If I were you I would experiment with nutrition, you never know, maybe the pure (muscle meat) carnivore diet is only almost optimal for you.

Good luck of course with the Endo!

Vitamin D levels and calcium absorption
When vitamin D levels are adequate, the body absorbs 30–40% of dietary calcium, compared to 10–15% without adequate vitamin D.

My GP ordered:

  • Calcium
  • Creatine
  • CBC W Automated Differential
  • Immunofixation, Kappa/Lambda light chain, Free with ratio serum
  • Protein Electrophoresis
  • Parathyroid hormone
  • Vacc pneumococcal conjugate

I’m fu*****. My endocrinologist just told me to stop my vitamin D supplements and retest 24 hour urinary calcium. The problem is anytime I stop the vitamin D supplement, I feel horrible.

My doctors checked parathyroid hormone and calcium, but they didn’t check vitamin D. I told him I was taking 600 units of vitamin D, if anything that’s not enough.

My vitamin D levels are 25 last time I checked.

Why he want me to induce a deficiency in vitamin D?

You were right. I feel as if recently being on TRT is the cause of the symptoms. My bizarre response to vitamin D supplements on TRT, as time goes on, I’m better able to tolerate vitamin D.

The fact that I’m able to tolerate more and more vitamin D as time goes on after stopping TRT is interesting. I wouldn’t be surprised if these symptoms abated in the next couple weeks.

TRT had a huge impact on the way my body utilized and stored iron, why not calcium as well?

The only far fetched idea (by correlation) I had, is that the oral formulation somehow changed something in your gastrointestinal system. Probably you had these symptomatic also with non-oral T formulations, therefore I didn't ask.

That is probably not the intention, more like establishing a baseline. Instead of adding something, cutting everything out that possible messes up your natural balance. Who knows where your vit D sweetspot is.

I’m thinking the same thing, the oral testosterone did change something about my body and the way it absorbs nutrients. I strongly believe it changed the uptake iron, vitamin D, calcium and God knows what else.

The symptoms are still present now only to a much lesser degree, and they seem to be continuing to decline with time. I’m able to tolerate more vitamin D supplement as time goes on and my requirements for vitamin D are going up.

I already went four weeks without vitamin D supplements, and I paid the price, my gums started bleeding and aching, and my teeth turned yellow, and I was becoming unable to perform my duties at work.

At 600 IU vitamin D3 it seems like barely enough.

If I stop my vitamin D supplements within a few days, my gums will start aching and bleeding, and my teeth will turn yellow and I will be incapable of performing my duties at work.

I told my endocrinologist I refused to stop my vitamin supplements.

I strongly feel as if I’m still underdosing on vitamin D supplements, the last thing I’m going to do is stop vitamin D.

What was his explanation of your symptoms? Thinking "Worst patient ever".

Have you considered the possibility that your urinary calcium is elevated due to your severely hypogonadal status? Low estrogen causes calcium loss in the urine, as your bones start disintegrating like a postmenopausal woman.

I have zero low estrogen symptoms. I’m sleeping well, actually my sleep quality is improving. I have more energy, erections improving, veins are huge, I’m getting stronger since increasing vitamin D from 400-600 IU but still feel it barely enough and I’m feeling better as time goes on.

This wouldn’t be happening if I was hypogonadal.

Also take notice that while my original T values seven years ago were 120 and 91 ng/dL in the early morning, my late afternoon T value is 104 ng/dL, hinting that my peak levels are higher if my tripling of my SHBG has anything to say about it.

My SHBG increased by triple the original value, it’s almost a certainty that my testosterone production went up along with it.

He listed several causes, hypercalciuria being one. I will admit I’m a difficult case. My body is highly reactive to substances even without the TRT.

The TRT takes things to an insane level.
