Some guys lack 5AR activity...some guys aromatase far more than others...that right there would be a thing for me personally; little DHT, lots of E. But DHT != prostate, hair, cardiovascular, or stroke.

My father has had DHT levels twice the top range all his life, is now 65 and has no health issue at all, completely normal prostate too.

He has E2 perfectly in range. E2 is what's causing prostate issues, not DHT

No, never had any prostate issues.

Cialis, manufactured by Eli Lilly and Co., was approved for the treatment of erectile dysfunction (ED ) in 2003. It can now be used to treat ED and prostate gland enlargement, or both if symptoms of the two conditions occur together, the FDA said.

This is an eye opener. I took Propecia starting in my late 30s for about 10 years and never thought of it as doing anything negative to me. Over that time I gained (more) weight and started having ED issues, but I figured the two were related to poor lifestyle choices. Never had a libido problem until more recently. Probably will never be able to disentangle what caused what, but now I'm thinking the Propecia certainly couldn't have helped things.

My urologist told me that handing a patient a propecia prescription is like handing him a revolver with one bullet loaded...it's a game of medical Russian Roulette. It may not cause a problem, but in far too many it produces catastrophic results. We've had a few members here dealing with the post-treatment syndrome it produces, and it's a thorny mess they are dealing with. Dr. Saya has posted that it's not a matter of cure, at this point it's a matter of managing the issues as best one can.

Sorry don't mean to question you but your post above almost implies you had a prostate issue. So I now assume that you take Tadalafil just for ED?
 
Sorry if it sounded that way. I should have said, since I started using Tadalafil, doctors started commenting how good my prostate felt. They never made that comment before I started using Tadalafil.
 
How do you know it is the E2 that is causing the problem?

Are side-effects of DHT like balding or acne are mostly genetics?

If someone have too high DHT but are not genetically prone to those side-effects, will those side-effects happen?

Or if someone is genetically prone to those side-effects, but his DHT is in range, will those side-effects still happen?

My DHT after 3 months on TRT is 125 ng/dl. Total Testosterone 1255 ng/dl. Dr Saya said the my DHT level is proportionate to where my Testosterone is, so it isn't concerning, unless I experience hair loss. But then I read here the ideal is 40-70 ng/dl. Does this mean too high DHT is ok as long as one doesn't experience negative side-effects?

So far I have no hair loss. Just bit of acnes. And faster hair growth.

My regimen, both 2x/week :
Testosterone cypionate 70 mg
HCG 400 IU

Can only comment on my experience, but while on injections my DHT was always up near 100 or a little over like yours. I never experienced any hair issues, or other problems with high DHT. Didn’t even have acne. I’m on the cream now, applying a few clicks to my scrotum, so DHT is most likely even higher for me, and haven’t experienced any issues yet.

My DHT level was 39 before this change which is on the lower end of the range. Dr Saya said it was low relative to my Free T level of 25.5. I’ve noticed MUCH better libido and erectile function. Better mood. Confidence. Deeper voice. DHT is critical.

I do cycle derivatives but the heightened DHT has given me consistant results over the last 8 months. As with all there is a balance though, I made some rushed adjustments and it bit me in the ass. Stay on course with minor adjustments with a long distance vision is paramount......

I agree. Too much DHT is ungood. I was on cream only (200mg/ml) and couldn’t tolerate the high levels and sides. I also recently added 25 mgs pregnenolone ED in an effort to raise LOW progesterone. Maybe that is helping too. ‍♂️

I created an account to ask you this question.

Very brief background: 5 years ago I successfully quit an opiate addiction that started when I was in high school. I began dealing with the Low T life immediately then at the age of 23. Around some point, I began self-administering TRT and eventually turned my life around thanks to it. Current protocol is 17 mg Test E ED with 120 iu HCG ED. I am a successful student with a 3.9 GPA about to enter nursing school for my RN. I do have energy and more cognitive focus than the large majority of those I encounter.

There is one caveat and I believe it is that HCG and Test alone are leaving me with sub-optimal DHT levels. Libido and erection quality is sub-par.

I just began microdosing proviron for this purpose. Can you give me any info on your DHT derivative protocol? If so, what do you use, and at which dosage?

Really interested to hear from you.
