I'll try to go through some of this tonight and see if there's anything I can add to the other knowledgeable members who have commented ...
With high E2 you will feel terrible. Because it negates all the effects from testosterone whether you are on it or not. I had high E2 like yours before I started TRT and I felt totally drained just like having CFS (chronic fatigue syndrome) So until you get your E2 to normal levels you are fighting a losing battle as far as feeling normal. I would start with getting this under control first. Thenthe next step perhaps could be to raise your FT3 slightly. Do one thing at a time until you stabilise what your working on. It'll take time to do this so don't rush it.
High E2 is a funny thing. I was on 1mg arimidex daily for a year and felt absolutely great and all of a sudden I came down with low E2 symptoms like sore joints and other stuff. I had to cut back significantly to get normal again. My shortcoming was that being in canada we couldn't get the sensitive E2 test. In your case you just have to get labs more often to see where you are. So yes it can take some time with higher doses and then it can change quickly once your body stabilises. Getting lab tests when you feel you are going downhill helps put you back on track. Getting E2 under control is the hardest thing I ever went thru.
If your thyroid is off it isn't the end of the world many people live long lives with obvious symptoms of low thyroid... you can live with that if you have to but living with high E2 is hell. I would prioritise the E2 first. Along with that will be the amount of T you take because you are at supra physiological levels. So getting your E2 down to normal range will mean lowering your T as well. More labs...
Another informative site a patient alerted me to just a few weeks ago and (WOW) the community has collectively supplied a goldmine of information and recommendations:

A LOT of moving parts.
"Currently - 220 mg test cyp weekly / 900 iu hcg weekly both of those are total numbers but they are split up to 3x a week. Test cream 3 clicks daily."
Are you taking BOTH "T" injections AND "T" cream????
If so, what is the strength of the "T" cream?
