Hi guys. New here. 32 y/o. I recently had some testosterone blood work done and my total T was at 267. My endo said since its still within the normal ranges i don't need trt. But i feel tired fatigued all the time and i feel i need it. Firstly thoughts on that ? Secondly which way would be the best route to go, oral or injectables and thirdly i have high ldl so should is that going to be elevated as i read somewhere that it will ?

Your endocrinologist is ignorant by choice, according to the endocrine Society guidelines, <300 ng/dL, you qualify for TRT. The most likely scenario is this endocrinologist isn’t up-to-date on his knowledge of male hormones and isn’t aware of the guidelines and doesn’t typically treat men with low testosterone.

This would indicate he’s into treating diabetes and thyroid matters only which is very common. Finding a doctor that knows how to properly diagnose and treat low testosterone may be a significant challenge for you.

You may have to seek care at a private clinic and pay cash for treatment.

Secondly which way would be the best route to go, oral or injectables
I’m on oral testosterone and I love it, 7 days to stable blood levels of testosterone versus 4 to 6 weeks on a roller coaster on injections. The oral testosterone route will typically have less side effects than injections while injections would be more convenient if you were injecting once or twice a week.

It may be more difficult for your insurance company to cover oral testosterone versus injections.

There’s no one-size fits all best treatment, you have to find out what works best for you.

Hey Nelson. Firstly thanks for getting back, i appreciate it. I used to live in Chicago but i recently moved to Asia. I'M single with no kids. And yes, I'd like to remain fertile. For me its baffling because my libido is low, my body hair, beard hair falls, im always fatigued amd tired. My body feels sore. I had my T levels checked in 2019 it was 280, same thing was said that they're still in the normal range so no TRT needed. Then i got it checked again in 2022 it was 320. Then again in 2024 267. Its never gone above 320 in 5 years now. A few thing's i would like to point out. My Ldl and serum cholesterol is high and i do have high Homocysteine as well. And slightly overweight. That's about it.

It sounds like you really should be on TRT by not only your numbers but also your symptoms.
