Blood Work - Comments Requested

Today, emailed back and forth with one of Defy staff member,Adam. I have done most of the following lab work on 09-21-2015 and 11-16-2015. I have done my Physical Exam on 09-21-2015. Some of my lab work is a bit over 90, but I haven't taken any medicines except fish oil.

Now I need to scan a copy of my ID and I am all set

I just got back from My Primary doctor's office with a PE request form filled out. I have forwarded it to Adam at Defy Medical. I don't write a lot but he misread my questions.
