Opinions on my lab results?

Background - 40 years old, BMI: 21, exercise on a regular basis, not taking any supplements or medications.

I would love to hear your opinions on my current lab results. I'm a bit concerned my test is lower than last years levels I also have results of my CMP and CBC.

Current Last year

Testosterone 387ng/dl 620 mg/dl (range 300-1080)

SHBG 44 nmo/L 51 nmo/L (range 77-80)

HbA1c 4.8, 5.2

TSH 1.32 micro-iu/ML 1.94 micro-iu/ML

Cholesterol 241 mg/dl 213 mg/dl (range 120-200)

Trig 67 mg/dl 82 mg/dl (range 40-140)

HDL 42 mg/dl 35 mg/dl (range 23-92)

LDL 186 mg/dl 186 mg/dl (range 0-130)

Vit B 792 pg/ml didn't get tested (range 180-914)

Vit D 63.3 ng/ml 62 ng/ml (range 30-80)

Welcome to Excelmale. You are a young man, presenting with very low levels of testosterone. There are tests that weren't done on the thyroid front that are important, and your LH and FSH values are not here - those are esssential. You said nothing about how you feel. Something suggested to you that your hormone levels were low, can you expand on that?

Generally, I'm healthy. The two issues I have are a lower libido compared to last year and I'm getting more tired at the end of the day. My DR says everything looks good (besides the higher cholesterol) and no other action is required at this time. I feel something is lacking or missing in my health and I believe perhaps is my lower testosterone. Unfortunately, the only thyroid test my DR ordered is the TSH.

Your doctor needs to dig deeper. He's misinformed if he believes a man your age "looks good" with a total testosterone of 387. He looked at the range and thought, "normal," and sent you on your way. Something is going on. To find out what that might be you need labs. If your physician won't run them, and don't be surprised if he won't, you can turn to www.discountedlabs.com. All testing is through LabCorp; many of us rely on it. It's fast, reasonably cheap, and comprehensive.

You need LH and FSH, to see if you are a case of primary or secondary hypogonadism. You need free t3, free t4, and both thyroid antibody panels (rt3 is needed but is a bit expensive). Prolactin. DHT, DHEA, and PSA.

Have you ever injured your testicles? Had a head injury? Used accutane, propecia/finasteride, or anabolic steroids?

I have really good insurance and I never pay for any blood test. The problem will be getting the DR order the tests. I asked him for a DHEA test while he was ordering the testosterone test. He told me he couldn't justify ordering the DHEA test because everyones levels go down with age and even if mine is low, there's nothing he can do about it. As for the other tests, I can always ask him and see what he says. Do you think an Endocrinologist would be more willing to order the more comprehensive testing if my GP isn't willing to do so? No, I haven't had any injuries to my testicles nor head. I've never used any medications or steroids besides antibiotics and that was years ago. I don't smoke, never drink alcohol, smoke or do any illegal drugs.

You are exactly where I was at about 6 years ago. Was feeling down, no energy, no motivation, lost sex drive and performance was dropping. Dr said the same thing. Your numbers are in range - mine T values were similar to yours. I even said to the Dr. that the T range covers men to 75 years old and don't I seem young (I was 42) to have T level of a 70 year old. He just hemmed and hawed about it said I was getting older. I got frustrated and felt that Drs were not going to help and did not go to a Dr for about 5 years. I was not mad, I just did not see the point of going if he would not help. That was the wrong decision so don't do what I did. In my case, my iron levels were too high and I had low T. I finally gave another Dr a try and he has been great - life changing. You owe it to yourself to find out what is wrong. I feel better now than I did 8 years ago. Good luck.
