BTW, adding cream into the mix made a DRAMATIC improvement for me. I encourage you to give it a try.
You mind posting your full protocol, with doses and timing and cream application site? Curious to see what a combo protocol looks like. Thanks.

Sure. I inject 24mg T Cypionate on M,W,F. PLUS 25 units of HCG on Mon and Fri. On a daily basis I use 1 click on the scrotum in the morning and 1 click on my forearms in the evening.
Awesome, thanks for posting that. Is one click 50mg?

I think that is true. The reason I don't know for sure is that there is nothing on the package that says how much a "click" is. A couple of years ago I called Empower to ask. Pretty sure that 50mg is what he told me.
Oh it’s Empower? Ya I’m pretty sure the cream they offer is 50mg per click.
Would you mind elaborating a bit for me? I have been on TRT for about 10 years and nothing has helped me. Tuned up on T, Thyroid, HC. E2 in check, was borderline for awhile, but AI made me worse. Have tried DHEA, Preg (oral and TD) and nothing has given me libido back or improved ED. Viagra, Cialis don't help much. Was a patient of Dr Crisler and considering Defy now. thanks.

Sure. I started on injections at various doses and various schedules, 1x per week, 2x per week, 3x, even daily. I always felt inconsistent. Some days fatigued and no libido. Some days raging libido and a feeling of being too amped up, coupled with insomnia. I also do not aromatize very much. In order to get my E2 over 20 it seemed like I would have to take too much cypionate. (I can tell you this much. Having too much T feels much worse than having too little.) I just never felt exactly right on the shots alone. After 3 years of messing around with different cypionate protocols, Defy suggested adding cream to the mix. The cream has increased my E2 into the 30's, without the "too amped up" feeling. For whatever reason, adding the daily dose of cream has steadied out the feeling I get from the testosterone. For a while I tried the cream without injections and that never felt quite right either. On the combo I get more consistent sex drive and a more consistent feeling of wellness overall.
I can also use the cream to micromanage the effect of T. If I feel like I am not quite getting enough effect, I can use more on my testicles and less on my arms. The testis absorb much more than the arms do. So I can change the amount of T I absorb by changing where I apply it. Also, sometimes I use less or more than the usual 2 clicks per day. The cream is processed by your body much quicker than cypionate. So these kinds of impromptu dose changes are easy to implement.

That's impressive that that doctor goes the extra step to ensure quality. You like to see a practitioner go the extra mile for his clients.
I use Defy. I would not be surprised if they require this as well. I have had good luck with both Empower and Hallendale creams. Both creams perform well for me and seem equally as powerful and each new batch seems to be consistent with the prior ones. I don't experience any symptoms when transitioning to a new batch.

I feel the same! I notice my libido and extra firm erections with cream (I’m assuming the DHT elevated helps) added into my protocol of injections which are 3x a week. Cream 25mg am 1x day. Initially they had me only adding cream 2-3x a week (scrotal application) but ea day made the difference overall best I’ve felt on trt (curious what your cream mg is ea day)

Why don’t you just put it all on your scrotum? Trying to keep DHT from going too high?

Correct. There's the DHT angle and then there's also the sex drive issue. If I use the full 2 clicks on my balls my sex drive just gets to be too much. Worse than being a teenager again. It's a good problem to have of course. But it can be a bit much.
God that sounds so fun lol, but obviously I see how that could be a huge annoyance at the same time.

My wife does a pretty good job of keeping me satisfied, but she's not on hormones. I can't ask her to do me twice a day, every day! It's just not fair to her. We've settled into a 4 times per week routine that works well for both of us.
The good thing is, that I seem to feel as good no matter where I put the cream. I don't notice feeling any better or worse if I put the whole 2 clicks on my forearms or on my balls. The only difference is just in how horny I get.
I've been doing 1 click (50 mg) for about 5-6 weeks now, applied to scrotum. Tested total T only about 2 weeks ago and it was at 740. I FEEL better overall on the cream, with one major caveat that's popped up in the last couple weeks. I started getting softer erections and reduced penile sensitivity, along with getting a bit "sloshy" around the middle. I haven't tested e2 yet, but I've been at this long enough to know that's the first thing that happens when my e2 goes up. Im pretty sensitive to e2, and if it gets up in the high 30s or higher I don't do well. Last time I had sex i couldn't finish because of lack of sensitivity (got a good cardio session in and plowed my wife a new one, so at least that's a positive). I also haven't fared well in the past with higher T levels because it tends to aggravate pre existing anxiety, which has been much better on the cream. Im determined to get the cream to work, and I may switch spots or reduce the dose. We ll see..........
