Hard lump after SubQ injection?

About every 3rd injection I feel some sort of hard mass under my skin. What causes this and is there a technique that would prevent this knot-like feeling under my skin?

Im using a 28g 1/2in .5mL insulin needle for my injection of 80mg/.4mL TestC.
 
You may have already tried this, but if not, experiment with injecting in other locations. For example, if I do SubQ in my abdomen area I am likely to get a lump every 3-4 injections but if I do shoulders instead I never get a lump. Also massaging the area for a bit right after you inject can help.
 
I also inject in my shoulders and love it. I inject in front of a mirror, to make sure the syringe goes straight in.
