What if blood viscosity is increased but blood pressure is not affected? Are the blood vessels dilated to adapt to increased viscosity, or is some 'lubrication parameter' increased?

In relatively healthy individual yes there is some room to adapt.



For someone with significant hardening then not so much room. There's a bunch of other details in my other thread. I used to post details ad nauseum but will keep it high level unless you really want the nitty gritty. Good questions.

Did anyone here with high hematocrit from TRT measure their blood viscosity?
I haven't officially measured it using testing that hasn't been adopted in any widespread sense. I use the poor man's test when I give blood. If it is coming into the tube like water and the phlebotomist compliments me, I am happy. They can tell you the stories of morbid patients that have blood viscosity that is really thick. Think about the wear and tear on the endothelium in that case. Your body may accommodate for a while but then?

Correct. If blood vessel is compliant then it can compensate for higher viscosity. Without compliance (atheroschlerotic disease), absolutely more shear with higher viscosity which begets vicious cycle.

Competing effects:

Viscosity will eventually win out if high enough and for long enough by consuming physiogical reserves. High viscosity for sustained periods.....bad, based on what we know today.
 
Watched the videos, read the paper.
I can just report that my hematocrit is 54% and my resting BP is no more than 120/80.
That is encouraging; two people can have the same HCT and dramatically different whole blood viscosity. Why? They have very different plasma viscosities (inflammation, disease). Hct doesn't tell the whole story of whole blood viscosity.

This is fascinating work. thanks for posting. As one who nearly depleted my iron, it was a stupid decision I made because a primary care doc, not my HRT provider, convinced me I was going to have a stroke if my HCT didn't come down. He then sent me to hematologist, who was more knowledgeable, and didn't think I even needed to donate with an HCT at 51%. Bottom line, my HRT provider that I should have trusted, was telling me the truth all along and not to worry about it until it gets to at least 54%. Since I now have my apnea under control, it has never risen to that level.
