Libido is controlled by the brain and lack of it indicates a problem in the brain, probably neurotransmitters. No wonder, libido is often restored to even hypersexuality with dopamine agonists. Everything else is secondary to dopamine.

Contemporary impotent medicine has almost zero understanding of the problem. Instead, we are served the usual fluff that it's all in your head and you should drink more water, sleep longer, and go to a psychotherapy LOL

TRT should help with relatively high T and DHT levels using e.g. T cream. It seems to take some time until there is an improvement.

I wonder if estrogen estetrol (E4) would help?

 
There have been countless posts on this forum from men on TRT who still lack libido including myself. TRT is only part of the libido puzzle (see Madman's diagram)
Most men who post on Male have issues. On average I don't believe most men on TRT don't have libido issues.

It's always comical on this forum when they claim that TRT solves all sexual problems. In reality, TRT resolves problems only when they were due to really low T before TRT, but many people get on TRT to "enhance" themselves, not because they actually had low T.

Testosterone is a weak manipulator of dopamine/libido. For example, TRT boosts libido typically during the initial honeymoon period but then the negative feedback kicks in, dopamine receptors downregulate, and libido goes back to zero.

If you want to enhance libido, it's more sensible to look into direct dopaminergic drugs, like the Amantadine study that I cited. Testosterone is a secondary drug and quite ineffective at manipulating dopamine, unless it was really low before TRT, causing low dopamine, in which case restoring the normal testosterone levels restores dopamine and libido.

I recall reading many of your posts. Are there any specific dopaminergic drugs that you had libido success with?
 
Guarana helps a little with dopamine (enjoyment and interest in life, libido, easier to get to orgasm) and it's not due to its caffeine component because pure caffeine doesn't do that.

I'm getting an Amantadine prescription soon. Pramipexole is another option that has been mentioned in the forum.
