Labs from 2/17/20
Total Test - > 1500 (264-916 ng/dL)
Free Test - 41.9 (8.7-25.1pg/mL)
Estadiol sensitive 42.8 (8.0 - 35.0)
SHBG 57.9 (16.5 - 55.9)
Free T3 - 4.7 (2.2-4.4)
DHEA 150 (31 - 701)
For these labs, my protocol was:
.4ml 200mg/ml Test Cyp & 400UI HCG 2x per week + 2 grains armour thyroid/35mcg cytomel

Labs from 5/29/20
Total test 1309
Free Test 15.3
Estradial sensitive - 28.7
(SHBG not taken, but likely high due to Total T/Free T ratio. LabCorp mistakenly missed lab)
T3 - 193 (71-180) (Free T3 not taken,LabCorp mistakenly missed Free T3)
Free T4 - 0.72 (0.82 - 1.77)
IGF-1 - 165 (88-246)
CRP - 45 (0-10)
Iron - 39 (38-169)

My protocol leading up to this was:
.1ml WEEKLY TOTAL 200mg/ml Test Cyp (split up in DAILY injections) + 2.5 grains armour thyroid/50mcg cytomel

With the high hs-CRP reading, I will include I exercise regularly, eat clean (many "anti-inflammatory foods") outside of artificial sweeteners, that I've recently eliminated. Take fish oil, and don't drink. One thing I'm not sure is related but will mention is that in 2018, I suffered a plantar fasciitis injury, that I still suffer from. That is around the time I began seeking hormone optimization due to weight gain and low libido.

I believe there is a problem going on here outside of my hormones. My main symptoms now are my inability to lose weight, despite training 6x per week and calculated calories, and I have no libido. The sleep and energy has improved (I’ve been less burnt out from work, which likely is to credit). The CRP suggest inflammation. I don’t know what affects this may be having, or remedies there are out there for this. The low iron is surprising, since I eat A LOT of meat. Seems like something is not being absorbed? The low T4 is odd, as I take 2.5 grains of armour daily, and in the past, it has been higher (but not optimal) with less of a dose.

I began having these symptoms in 2018, and my initial labs then did show low T/low E/High SHBG/Not optimal thyroid. Therefore, I thought this treatment was the answer to the weight and libido. However, with my numbers looking good and me still having these issues, I feel like there has to be a different culprit. Any insights on what this may be, and the remedy for it, would be great. I will mention I eat clean, dont drink/do drugs, and exercise reguarly.

I’ve been on TRT & Thyroid treatment since December 2019 (was just thyroid prior to that). Previously, my numbers were looking good, but I still had symptoms (Weight gain, low libido, bad sleep, low energy). I was encouraged to drop HCG to “dial in”, and have been without HCG for 10 months, and just Testosterone Cyp (1ml weekly, split up in daily injections).

At this point, I have an appointment with a Functional medicine doctor, as I feel there must be something else going on. This may be a longshot, but I will also say that I urinate more than anyone I’ve ever known (up to 4 times an hour). I’m seeing a urologist about this. I’m thinking maybe there is something going on there that play into my current symptoms as well?

Any insights are appreciated.

I'm on NDT plus Cytomel as well.
When I added in iodine, I noticed much better absorption with my thyroid meds.
I personally take 3mg of Terry's Naturally Trio-Iodine.

Also, on your second labs it looks like your FreeT dropped pretty significantly??
I realize your TotalT is high, but I'd be more concerned with giving that FreeT a decent sized bump.
Also, we definitely need to see FreeT3.

Are you donating blood frequently??

He should be more concerned with getting his FT tested using an accurate assay (equilibrium dialysis or Ultrafiltration)and definitely not relying on the direct immunoassay especially in a case of one having high SHBG.

Doubtful anyone with high SHBG running a TT 1300 has less than stellar FT level.

His ferritin is most likely crashed/low which would definitely hinder optimal thyroid function.

I donate blood maybe 2 times a year. I am already taking Iodine. I'm taking Boron to hopefully lower SHBG, but I've taken that for awhile, with no significant decrease.

I'm adding back the HCG, and lowering the Test to .90ml per week, since dropping HCG didn't seem to make a difference, and fertility is important to me.

For the high CRP, the doctor is prescribing me Low Dose Naltrexone (4.5mg dalily). I've researched this and it seems to only help 20-40%, but those who it does help, benefit significantly. I'm curious if anyone has any experience with this, or any other treatments for inflammation?

None of the test you mentioned are likely to be connected to the high CRP.

PSA, LDL,liver enzymes, cancer, infection, inflammatory bowel disease, lupus, rheumatoid arthritis amongst many others are possible sources of the high CRP.

I would assume you already tested PSA, LDL, liver enzymes, is that correct?

Testing ESR rate and other inflammatory markers would be my choice of action, trying to get at the source of your inflammation rather than trying to suppress it.

It's a full time job trying to figure it out, in these situations a good doctor might be better able to help figure it out.

Do you know what your CRP was prior to your plantar fasciitis injury?

I couldn't find my more recent previous lab result. I just found it. Keep in mind one is from Quest labs, and one is from LabCorp, and the injections prior to the readings.

Labs from 3/24/20 (From Quest. Last T/HCG injection was 6 days prior)
Total Test - > 1322 (250 - 1100 ng/dL)
Free Test - 101.7 (46-224pg/mL)
Estadiol sensitive 42.8 (8.0 - 35.0)
SHBG - 74 (16.5 - 55.9)
Free T4 - 0.8 (0,8 - 1.8)
Free T3 - 3.0 (2.3-4.2)
For these labs, my protocol was:
.4ml 200mg/ml Test Cyp & 400UI HCG 2x per week + 2 grains armour thyroid/35mcg cytomel

Labs from 5/29/20 (From LabCorp. Last T injection was 4 days prior)
Total test 1309 (264-916 ng/dL)
Free Test 15.3 (8.7-25.1pg/mL)
Estradial sensitive - 28.7 (8.0 - 35.0)
(SHBG not taken, but likely high due to Total T/Free T ratio. LabCorp mistakenly missed lab)
T3 - 193 (71-180) (Free T3 not taken,LabCorp mistakenly missed Free T3)
Free T4 - 0.72 (0.82 - 1.77)
IGF-1 - 165 (88-246)
CRP - 45 (0-10)
Homocysteine - 10 (0.0-14.5)
Iron - 39 (38-169)
My protocol leading up to this was:
1ml WEEKLY TOTAL 200mg/ml Test Cyp (split up in DAILY injections) + 2.5 grains armour thyroid/50mcg cytomel

I don't know what my CPR was beore, as this was the first time this was tested. I don't know my ESR, but my PSA and LDL is in normal range.
