I was recently prescribed cialis 2.5mg daily for E.D. I also have a recent diabetes diagnosis and I’m working on weight loss to try to reverse it. Although I feel like an old grandpa taking ED meds, truthfully I need it (especially after my endo’s failed experiment with clomid and stopping TRT… but that’s a different story!).

Anyway, after a week, I don’t think I’ve seen any difference. I am going to ask my doctor to up my dose to 5mg. I am wondering though- is it better to take 5mg at night, or split the dosage and take half in the morning and half at night? Though sex happens mostly at night, there are times when the wife will want it during the day!! ;)

Had anyone here had experience with cialis twice daily vs. once daily?

Anyone here with diabetes and ED - did cialis work for you, and at what dose?

People with uncontrolled diabetes, are typically going to need higher doses of Cialis. I over-respond to most medications and 2.5 mg Cialis daily is not enough.

I have type 2 diabetes, and after two weeks on 5 mg Cialis, I noticed, whenever I would go to itch the bottom of my foot, I would end up tickling myself. My veins are bigger, and my energy levels are higher giving me insane pump in the gym.

Yes, I had a degree of ED, Cialis to the rescue. It even fixed my ejaculation and urination problems.

Cialis reverses endothelial dysfunction, treats diabetic neuropathy, and makes you more sensitive to insulin. Cialis basically reverses the damage caused by diabetes.

Twice daily dosing isn’t necessary because the half life of Cialis is 36 hours.

Thanks @Systemlord - that's really good input. I just finished diabetes education classes and learned about all the things that it can mess up on your body... the penis being one of them. :( I'll definitely give 5mg daily Cialis a try. Thanks!

Once daily is all that is needed due to the PK.

* peak of the serum concentration is reached about 2 h after the ingestion
* t½ is around 17.5 h
*
chronic administration of tadalafil enhances erectile function for up to 36 h

Even though it can be taken with food it is best to take it on an empty stomach 2 hrs before sexual activity.

Keep in mind that having healthy testosterone levels will improve one's response to PDE5i.

Tadalafil

Tadalafil was approved in the United States in November 2003 [15]. It has particular pharmacokinetic properties. Absorption does not appear to be influenced by the intake of fatty meals or alcohol [16]. The peak of the serum concentration is reached about 2 h after the ingestion different from sildenafil, vardenafil, and avanafil which require 1 h. In addition, the half-life of the molecule (t½) is around 17.5 h [17, 18]. Therefore, a chronic administration of tadalafil enhances erectile function for up to 36 h and may restore more physiological sexual intercourse [19, 20]. However, tadalafil's prolonged half-life has a higher risk of long-lasting adverse effects than other PDE5-Is. Indeed up to 30% of men taking tadalafil report side effects lasting longer than 1–2 h [21]

Tadalafil (generic version of Cialis) is good stuff, it helps a number of mens issues. However, it is very long acting, so while you might see a benefit an hour after taking vs. 10 hours for example, it's not likely to be enough of a benefit to be concerning. if you need a bigger dose, taking it BID or twice a day, isn't really going to help much over taking a larger dose once a day. Just get your doc to buy in and go to the 5 mg dose, and if it turns out you need 10 mg a day, slowly titrate up to it to avoid side effects. Many men now take it 10 mg a day without issue, but, again titrate (gradually increase the dose) don't just go from 2.5mg to 10mg.

I took diabetes education classes as well, some of the things we’re taught caused me to cringe, and one thing I wholehearted disagree with is the goal of getting your A1C <6.9.

Not good enough!
Diabetes is caused by a fatty liver and fatty pancreas. The fat develops from long term caloric surplus, or high fructose, beer etc … people with hard pot bellies are a tale tell of fatty liver . Look Newcastle study … I’ve done Keto , Carnivore, vegan , vegetarian, fasting, testosterone, hcg .. over the last 7 yrs , trying to find health optimization. Nothing has worked for my belly fat and body composition like calorie restriction . I also do not eat processed foods , and no bread … I’m 57 about 14% body fat .. Cialis won’t help visceral fat.

How did you develop diabetes was it just something that was almost inevitable or other lifestyle factors?

Good ole not eating so darn much. How about that!

I've learned a lot of good things in diabetes education classes, but also a lot of information that conflicts with what the doctors tell me. I don't know if that's a matter of the class having outdated information, or the doctors having a different opinion or just not enough up to date information on diabetes. Regardless, I still think the classes were helpful. I agree with @Systemlord though that what they teach about "acceptable" glucose levels for people with diabetes is really quite unacceptable. That's why I'm enrolled in a weight loss program, to get my a1c to normal levels (under 5.6) so I can "reverse" diabetes (put it in remission). I'm at 5.8 now... down from 7.6.

For me, I think getting diabetes was a combination of lifestyle and genetics as it runs in my family. It looks like I had pre-diabetes for a few years but since I didn't have a PCP for 2-3 years, nobody told me anything. However, a strong steroid treatment for asthma during an influenza episode last year pushed me over the edge... from pre-diabetes to full-blown, uncontrolled diabetes. It felt like a life sentence at the time, but little by little I'm improving and getting out of this hole.

Thanks all for the advice on Cialis. I am seeing the doctor this week and will discuss increasing the dosage to 5mg. I will try splitting it morning/night for a few days, then I'll try just a full dose at night for a few more days, and see if there's a difference at all.

Thanks!

Poor diet, lack of exercise and being heavily medicated, sedated for 30 years didn’t help matters.

The crazy part, I didn’t get to see the normal size of my penis, fully flacid until age 45! Then a month and a half later my doctor forced me off a benzo which lead to protracted withdrawal, and now I need TRT.

Type two diabetes runs in the family, my dad has it. He first developed it when he stopped running at age 80.

Wow Im so sorry to hear that must have been such a rough ride. And wow you dad got it as soon as he stopped running. Scary stuff that diabetes. I have an a1c test coming up. fingers crossed.

Pretty close match to what is described as anti-aging protocols. And it restores morning wood making me like a child with a new toy.

Diabetes is caused by years over consuming more energy than the body can burn . Whether that is from fat , carbs , protein, it doesn’t matter . Once the body passes its fat storage threshold, you get diabetes .
