TRT Numbers

Ok, androgel didn’t seem to work. My testesterone numbers were around 300. I’ve restarted it because i was feeling badly. I feel better since taking it. The “300,” is that always accurate. I know there’s another measurement (free testesterone?). Is it posssible the two measurements could be different. In other words, could the trt be working even though the number above is low? I’m set to see a urologist next month.

With topical testosterone, it all depends on when you had your bloodwork done. 12 hours after application? 24 hours after application? Your testosterone will go up quickly after androgel application and then fall throughout the day.

TRT isn't working if you tested between 4-6 hours after dosing. TRT will be working when your TT and FT are near the top end or above the ranges.

It may be time for injections injected weekly or twice weekly or oral T Jatenzo or Kyzatrex. If your urologist suggests injections every 2 weeks, ask if you can inject 2 times weekly at a lower dose (50 mg per injection) and if he doesn't go for it, politely decline and search for another doctor.

Usually next day, maybe 18 hours after application.

You did it wrong, no wonder your results are low. Back to square one, retest between 4-6 hours after application.

Your TT and FT levels will rise within 2 hours after application and decline quickly. I was on oral T and half-life is identical to topical T. I would peak at 1052 ng/dL 2 hours after consuming the oral T capsule and would see levels nearing 300 at 12 hours.

The gel results in high psa numbers. Would the injections do the same?

Gels are known to increase DHT way more than with injections. DHT can be in 5000-8000 ranges in some cases on topicals while injections would see slightly out of ranges (100>)

DHT can interfere with the prostate in aging men. I would highly recommend against using DHT blocker medicines!

Is it possible that the testosterone is working even though the numbers are low. In other words, could the “free” testosterone be high even though the normal number is low? I do see some good changes.

Again, your numbers were low because you waited so long to test. On injections it is standard protocol to test at the lowest point to see what your trough is. On topicals it is standard protocol to test at the highest point. With such a short-lived peak you need to know what the peak is and dial in that way. To do that, you’d need to test around 4 hours after application as Systemlord stated above. You waited until your body had already processed most of the test, so of course the levels would come back low. For example, I’m on injections. The half-life is between 5-7 days. So one week after I inject I only have half(50%) of that dose left in my body. In two weeks I only have 25%. I wouldn’t wait two to three weeks after an injection to test my levels because I know they’d be low due to already having most of it removed. That’s basically what you did with your testing.

It’s possible you are seeing good changes because you are hitting decent levels for part of the day. However, gel is a suboptimal delivery method, with injections and creams being much better options. Even oral testosterone is gaining traction these days, though the cost may be an issue there.

This time apply the gel before your appointment in the morning. You could also ask to test your DHT, it might be just fine contrary to what others claim. It's not proven that one needs high end or above T levels for successful TRT.

You're probably using androgel 1.62%? There is actually pharmacology claiming the levels with gels are not fluctuating as much as one would think. One exception is testavan gel that peaks fast.

I only got 2 pumps prescribed. My TT was around 300 24h post application. Based on the age stratified TT levels, here they say everything is fine. ....
We had here posts, where they measured after 24h and there was a big drop in levels, contrary to the studies. That's why I suggested to apply in the morning and measure the AM T level.
81mg is a 4 pumps dose and that's a lot to disperse on shoulders and chest.
