My husband (THANKS TO THIS FORUM and the gentlemen on it) has his TRT dialed in nicely, his libido is good, he's being treated for severe sleep apnea going on 2 months now which i think is probably the most important thing to happen to him..he had around 65 episodes an hour and he's now down to an average of maybe 14...hopefully his body will start to heal and i imagine it contributed to his low T and his ED. Anyway....

Been perusing the forums extensively especially for treatment of his ED. He has been taking daily Tadalafil (10 mg, we get it from ADC) and also takes it for BPH but still needs generic Viagra on those days we have sex. He is still really not happy at all with the quality of erection although it's enough for penetration. I really think we need to treat his body for this, not just a bandaid like Viagra although we're thankful for it.

I had bought some Citrulline months back and forgot about it. Started giving him around 6 g's daily along with some grapeseed extract ...should i buy the more expensive Pycnogenol or is GSE acceptable?

I also have a question about what i could add in the "stack"? i've looked at Gene's stack and haven't explored all of what is in it and i'm leary about adding any more "drugs". Am looking at beet juice extract maybe??
Also..he is on Lasix for swelling of the lower legs probably due to mild non-alcoholic liver cirrhosis and i was concerned about low BP with Tadalafil and the addition of Citrulline so i'm taking him off it temporarily to see if he fares well w/o Lasix.

Googling Nitric Oxide and found out it's important in anti-aging which is another huge reason for supplementation. My question to you all is what else that isn't very expensive can i add to the Tadalafil/Citrulline/GSE (or Pycnogenol)? Also...is there immediate benefits or is it more accumulative? Reason i ask is because i want to take Citrulline to see if it helps the quality of my orgasms but i don't know if i should be taking it daily for that? And how much can women take daily for benefit, would i have to take the 6 grams?

(Reading a post below..) i forgot that he has Tamulosin for BPH. We had stoppped that almost a year ago because we thought it might be contributing to lowered libido (but don't know) and i see it's a factor in the "stack". H's doc told him to take it away from Viagra and Cialis because of the BP lowering effects. I just put him back on it (at night) because he does have trouble with peeing and it helps so maybe i should just keep him on it and see how this all plays out? SO sorry this post is so long.

That is a big topic but here are a few ideas:
- I have found beet root powder in bulk to be the best NO booster I have used (I am assuming NO is its MOA). Jerry Brainum has a good video on NO boosters and he is a big supporter of it.
- There was a recent post here about DHEA possibly having an NO aspect and that is consistent with my experience. It is apparently helpful for women too, but more due to its conversion to T in women.
- Sensible sun exposure apparently has an NO boosting aspect, and that, in men, may actually be part of why PT-141 works since it is basically activating the melanocortin system for which there is evidence that it has a vascular health aspect..it is like an injection of sun exposure (a really large one) and as discussed elsewhere is now officially approved for women, although at a dosage which IME experience would be at least 7 times too high for men to start with.You could try it together.
- Have you read the "Ultimate adrenergic control of erections" thread? That has lots to say about various alpha-blockers as well as lots of other fascinating stuff.
- And while it it requires a degree of health and most people don't like to hear this, I'm convinced that sprinting (ideally) or at least HIIT training is likely a big NO booster. Most people seem to over-focus on pills and potions and under-focus on developing physical work capacity, which is a large part of vascular health IME. And it's close to free. Anythng that develops cardiac capacity without overtraining is likely to be helpful, such as long walks for example.
- While not directly NO related, have you looked into the DHT boosting? Many of us here think it is a huge and underappreciated factor.
- Personally, I never felt that L-citrulline did much. I think I got more out of L-Carnitine Tartrate, but that's just me.

i will take more time to ponder all that you have said. He is on DHEA, a modest amount, about 25 mg. i can only take around 5 mg and it does have libido boosting effects. I definitely do cardio and some HIT but i have to be careful because of joint issues.

Could i quickly ask what brand of Beet root powder do you use and how much?

Great info in the post by Guided by Voices. I would also highly recommend raw beets, a lot of them, and if you don’t have the time to cut and peel then use the beet root powder. I use both a lot and they really do help with NO. Also celery, arugula, kale and spinach. Beets seem to be the most potent for me though. One tip is to not use mouthwash, it will diminish your ability to digest nitrates from food since it begins with saliva. L-Citrulline has really helped me too at only 1000 mg per serving so that’s in my toolbox too. I think as we age we need to seriously embrace all these things that boost NO from sunlight to diet to supplements. Pycnogenol is on my list of things to try. You’re a great advocate for your husbands health.

Thank you! Yes i read that about mouth wash, very interesting and i absolutely do believe in boosting NO in older people after what i've read. I would like to combine beet powder w/citrulline and see how it goes, there are so many different brands on Amazon

I use the bulk supplements brand, and a teaspoon at a time should be more than enough. An advantage of beets/powder seems to be that they have a long-lasting affect.

Also, some people report benefit from a combination of high-dose vitamin C and garlic, as well as pomegranate, so those are worth looking into as well.

Thank you, just ordered. I do have Bulk Supplements Citrulline, we'll see what happens. At any rate, i know it's good for us.

Oh, doing more reading on NO... Since he will be taking Citrulline (malate) and now Beet juice powder (he also takes pine bark extract with Citrulline) should we stack that with either the addition of Arginine or try L Carnitine? (He also takes daily Cialis...thinking we should space these apart to avoid possible low BP reaction).

I don't have a strong view on Arginine. I never noticed it doing much for me and I seem to remember that it is (in theory) less effective than Citrulline for some reason, but as with almost everything else, there is no substitute for trial and error. There is probably also a point of diminishing returns at some point. The advantage to Carnitine is that, if I remember correctly, it works through a somewhat different pathway, but I would have to go back and research it again to remember the details.

Have your husband been checked up by an urologist to rule out any phsycial issues?
Sexual performance anxiety is a very common cause and it can even show up after many years in a longterm relationship. Meaning its more a common in the beginning of a sexual relationsip but it can for sure happens after many years as well.

Gradually introduce new supplements into the routine. It's the most prudent method of determining what works (or doesn't).
