A bit confused by these results. I think I was being mistreated on BioTE pellets and have converted to T-cyp 40mg IM M-W-F and HCG 500 IU T-Th-Sat.

Total test: 928
Free test: 12
Estradiol sensitive: 5.9
SHBG: 78
DHEA: 574

Why so low on the free testosterone? What to do about the SHBG?

I personally haven't seen a Total that high with such a low Free.
Only way to overcome that super high SHBG is to increase the dose.
You're gonna be running a fairly high total, but that's what's needed to push that SHBG down and drive that FreeT up.

That's non-existent estrogen.
 
I'd cut that way back if not remove completely.

Out of curiousity, do you have any thyroid numbers?
Would like to see what that's doing as well.
 
SHBG is astronomical, as Sean says you need to up the dose, your TT is probably going to have to be 1000+, 1100, or even 1200 to get Free T to come up.

I think that 1mg of Anastrozole is too much but too little info is present here to give an informed opinion, like when the blood draw occurred or if problems are present related to low (or high) Estrogen.
 
Short of a liver transplant, you'll need to throw more testosterone at the insanely high SHBG. You're going to need a Total T of 1500> to get Free T where it needs to be.

I have to run 1200 plus on my TT for get my FT around 30. IMO your E is way to low and might consider not using an AI.

What is it you have a hard time understanding regarding the newer research on SHBG:T binding?




An extremely high TT is not needed even with one having high SHBG. With a TT of 928 ng/dL, SHGB 78 nmol/L and Albumin 4.3 g/dL (mean) his FT using the newer calculated TruT method.....TruT Free Testosterone Calculator by FPT.....(which everyone should be using) is 27.71 ng/dL (above the mean 23.5 ng/dL of the reference range 16-31 ng/dL).

With a TT 928 ng/dL his FT levels are by no means low......let alone sub-par even with an SHBG of 78 nmol/L.




Sure he has room to easily bring his FT levels up higher (top of the reference range or above).....and if anything he would only need to hit a TT of 1000 ng/dl to have his FT at 30.39 ng/dL (top of the reference range) or hit a TT 1200 ng/dL to have his FT at 37.89 ng/dL (over the top of the reference range).

Nothing you can do about shbg. It may drop some over time with trt.

Ignore your free T test. That number is garbage.
I was on BioTE pellets for the last year with a total T of 1600. I hated getting the pellets and I fluctuated wildly with how I felt.
