I would agree with DG. While I certainly have my mental issues as it relates to ED, the majority of ED cases as we get older are vascular or hormonal causes. The psychogenic component can certainly make it worse.
Erection is under control by the nervous system. The vascular system is just executing commands. The nervous system gets damaged with aging just as much as the vascular system, but that is not discussed often. Damaged inervation to the penis is experienced as loss of erection and also as viable erection but "loss of sexual sensitivity" (different nerves) and is very common with aging. This is a different category from psychogenic or psychological loss of erection.
How many people that claim their ED is due to vascular problems have actually had an ultrasound to the penis blood vessels to confirm that?
Even if the problem is confirmed to be vascular, injecting PRP is just creating a temporary inflammation with the hope this will "rejuvenate" the penis blood vessels. The "results" of such procedures are vastly exaggerated and just a cash cow for the constantly failing "rejuvenation" industry.
Agree with your first paragraph. In a lot of aging men, that nerve damage is due to type 2 diabetes. Hypertension (like I have) can also damage the delicate endothelial lining even without plaque buildup and subsequently cause ED. Lots of causes, that all can intertwine.
