To the OP:
Won't pile on the smoking lecture, my mom is 80 and has been trying to quit for like 70 years, so I get it.
Two of the common issues with high blood pressure (even moderate hypertension like you reference) are endothelial dysfunction and stiffening of the artery walls. Many studies show that this is a likely cause of venous "insufficiency" in the penis, or in other words losing your erection too quickly. I have a very healthy lifestyle, but have some hereditary issues with hypertension. Mine is well controlled now with a moderate dose of Lisinopril, but it has certainly effected erection quality beginning in my late 40's (am now 54). Getting the BP under control and being on TRT has helped me immensely.
Given your hormone numbers are solid, I would focus on improving blood flow: Nitric Oxide boosting supplements, continue Cialis, lower BP (possibly with meds), and of course stop smoking if possible.
