I also ran my results in the free and bioavailable hormone calculator. According to this, my results are-

Free Testosterone- 9.34 ng/dL = 1.49 %
Total Testosterone- 234 ng/dL = 37.3 %

(Using my results of Albumin 4.6, SHBG 56.10, Total Test. 626.3)

Now what. My libido is in the tank, I have ED and my memory is shot.

Your SHBG is high but you can't really do any thing about that, creates the Free T problem you're having. Its a shaky spot to be in, some Drs (like Defy) may be willnig to treat you based on your Free T. Typically the administration of Testosterone can push the SHBG down to some degree making your Free T come up.

You have a minor FT3 problem but I would avoid stepping in to the hypoThyroid stuff.
 
Sadly the majority of so call doctors out there are profoundly ignorant when it comes to sex hormones, add to the fact that hormones are virtually ignored in medical school and residency and it's not hard to understand why doctors are so profoundly ignorant.

Men normally have between 2-3 percent of their testosterone free and bioavailable, yours is below the standard 2-3 percent and is a blunder on your doctors part. Testosterone binds to SHBG and when elevated it can negatively impact your free testosterone, if your doctor had run free testosterone, you would see it very low.
 
Your results look perfectly fine my brother.. Your DHEA levels are a bit up, but nothing to worry about.

You can try different things to get better erections, like 5mg ciales daily. And your free T3 can be also go up a bit, by just taking iodine and selenium. Usually just a good multi vitamin does the work.

Anyway, there are some supplements plants based, that might help to. Like long jack, tribulus and so on. For me, while ago, just some test boost helped me when I had any ED issue. Although it didn´t change my T levels, it gave more libido and better erections.

I would do everything, but not jumping in TRT, with those labs results. Just see how many guys dont do well on it, with that of numbers you have, there is no way need it.

However, some guys have a really heavy life style, by working a lot, taking care of family and so. For those, that even dont like the kind up´s and down´s of being natty, and have all these things to take care about, I´ve seen they all would prefer the stability of TRT over any kind of natural levels. If you feel you might need some help, just see a specialist and have a talk. Number´s are not everything, and some times is better to go with TRT, then take some other pill´s to "help" go through a hard time in life.
I have tried all those herbs and supplements you've mentioned. I'm still taking selenium and iodine. None of them have helped my libido or ED. I have had low total testosterone on tests before, including one from last month. I'm not sure just one normal total testosterone level shows that I don't have hormonal issues.

Many establish guidelines that way to authorize treatment ($$). Unfortunately in your particular situation your TT being way above 250 makes you not a TRT candidate in mainstream medicine.
When you're willing to go out of pocket with a place like Defy is how you get this treated.

Thanks. I actually did find a doctor who would treat me based off my 290 something total T but he would only give me pellets. I didn't wanna go there. Then this new DR said over the phone she would treat me but when I saw her she drew her own (very funny looking) labs. She claims her labs are in house and she does them herself. So not sure I even trust these results. From now on I'm insisting on only Quest/Labcorp.

Edit: Oh the icing on the cake was this doctor telling me I'm too young to have sleep apnea. Can't make this stuff up.

Are you doing anything to optimize your thyroid at the moment?
Seems like there's room for improvement there as well.

Nothing other than taking selenium, iodine and living clean (eat organic, no smoking/drinking etc) I've already seen a few doctors over the years who won't do anything for my thyroid. They say it's perfectly fine.
