Metformin is the most commonly prescribed oral medication for diabetes. It has side effects, like gastrointestinal distress. Other meds like pioglitazone remain in use by informed providers. Why? Pioglitazone has demonstrated decreased heart attack and stroke events. Pioglitazone gives better outcomes, but also has more serious side effects. It's similar to the ACEI vs ARB question: drugs that decrease cardiovascular inflammation improve outcomes. But they also have more side effects.

Ford Brewer is a physician that started as an Emergency Doctor. AFter seeing too many patients coming in dead from early heart attacks, he went to Johns Hopkins to learn Preventive Medicine.
