On case by case basis, some insurance companies do reimburse for Hormone Replacement Therapy and Testosterone Replacement Therapy. It is each patient's responsibility to contact their own insurance company to confirm reimbursement. Defy Medical will not contact your insurance company nor will take any responsibility for any additional forms your insurance requires. Defy Medical provides claim assistance by completing the standard claim form (CMS-1500) for the patient to submit to their insurance company.

Free Assessment: Yes

Airfare Credit: Not needed, we treat out of state patients in the comfort of their own home.

Medications shipped: Yes, directly to you from one of our licensed US Compounding pharmacies. You choose the shipping method at each refill.

Contract Free: There is no contract. You simply pay for the services and medications that you order. Patients may discontinue at any time and will be provided a free evaluation to provide assistance with coming of testosterone therapy.

Syringes: Syringes and other medical supplies can be ordered at anytime and will be provided with each refill.

Blood testing: $150 for the initial TRT panel and $145 for the follow up TRT panel. If you have insurance coverage for blood work, Defy Medical will provide a prescription to take to your own lab. Annual visits: after your initial consultation you will be required to complete blood work and consult the doctor twice per year (every 6 months).
