6 month bloodwork

Here are my results:

I'm 39 and been working out for for about 25 years now. Had some blood work last year after experiencing symptoms of low T. Started on 50 mg of test cypionate every Monday and Thursday.

Here are my levels and how they have changed:

Total test : 622 to 1181
Shgb :65 to 46.7
Estradiol :31 to 46
Free T :8.1 to 23.5
H/H :15.6/46.3 to 16.6/50.7
AST :23 to 39
ALT :32 to 50

I had the blood work done the day after my injection so my test levels were probably at their highest. I feel great since starting the injections. I've definitely gained muscle mass and strength. Sex drive is the best it has been in 10 years.

My main concerns are my liver and estradiol. I decided to start milk thistle but read articles saying elevated liver functions can be normal but are my levels ok ? Also the estradiol is it too high, high enough to warrant arimidex? I scheduled an appt to give blood next week since it seems that will be a regular appointment for me.

The is the wrong way to do things, you test at trough, not peak. Your levels are lower 2.5 days later and that's where you want to test levels.

Whatever is causing your liver enzymes to be elevated is probably the cause of your low-T, because TRT improves liver function.

Also the estradiol is it too high, high enough to warrant arimidex?
Your levels are too high for natural T levels, not for someone on TRT. The half-life of natural T is 30 minutes, the half-life of cypionate is 5-7 days.

This statement is worrisome. It's nice to help others in need, I donate every couple of months, but you need to reassess whether or not you're able to donate blood on a regular basis now that you're on TRT which can change things.

This is not normal procedure for everyone on TRT, so be careful what you choose to believe your doctor has told you.

Careful, excess blood donations can crash your ferritin levels and now that you're on TRT, this can also decrease your ferritin levels and before you know it all this success you have achieved on TRT could disappear.

1. I was unaware about drawing levels at trough but I'll know that going forward. This appt has been scheduled for months and I didn't want to adjust my dosing schedule for the appt.
2. I don't know about the liver enzymes because they have been normal for years until starting the TRT. Definitely saw an increase in AST and ALT but bilirubin was still the same level as prior to TRT. Started milk thistle and hope the increase is just a temporary increase that will level itself out.
3. Thanks for the video on estrogen my level seems to be just fine with the higher T level.
4. With a hematocrit of 50% you wouldn't recommend a blood donation? Seems like it has climbed up fairly quickly and just don't want to risk it getting too high and a blood bank not letting me donate.

No, I wouldn't recommended a blood donation for a HCT at 50%. If it bothers you go to a lab company with a 52% cutoff. My providers lab company has a 49% cutoff for HCT. I had an HCT at 55% and my doctor wasn't worried because my blood pressure was controlled.

It takes knowledgeable doctor with actual medical knowledge not to make knee-jerk reactions and take hasty actions that leads to a decline in a patient's health.

Of course your HCT increased, because your testosterone increased, both are benefits of TRT.

If they increased after the cypionate and milk thistle:
Local pharmacy cypionate? When did you start the milk thistle and is it a tested product?

AST:ALT ratio is <1 in both tests

What's the ALT lab range? Typically around <45 is considered normal.

I'd recommend sticking with certified supplements.
