On TRT for 3+ Years. Now I have extreme itching

Hey guys.....

So I started TRT about 3 years ago via a local clinic and ive always been a very good responder to the test. 200mg per week puts my levels in the 1000-1300 range. Last year, I wanted to donate blood to keep my RBC down and at that time I started some other brand of test that I got from another source. Soon after that I started getting itchiness all over my body but it wasn't that bad. I dealt with it for about 6 months till it got progressively worse.

Finally, about 45 days ago I stopped all test, went to an allergist and a blood doctor - the only suggestion was to stop the test so I immediately donated blood again to kick start the process, stopped the test and it got progressively better. I contacted a new doctor, got new pharma test about 10 days ago and started at a much lower dose of 100mg per week.........and boom....I got the itchiness back

******I want to note that this itching ONLY happens when I get hot......*******
again.

My question is, Has anyone had this happen? Should I be running Ethanate or Prop instead? I just cant take the idea of not being on test. My quality of life on ALL fronts is so much better.

This has been reported for people with PV, but many men on TRT share the same high hematocrit values as PV patients. I am just speculating. What is your hematocrit?

You could have also developed a reaction to the testosterone carrier oil.

Iron is blood and you’re bleeding that out for donation. You’re could be iron deficient. Now, if you’re indeed iron deficient and you start TRT, the itching would it be expected to get worse because higher testosterone can use up more iron in the creation of red blood cells exacerbating your iron deficiency and therefore decreasing the amount of oxygen to your tissue, thus causing the itching.

It could also be a reaction to the new brand of cypionate.

You’re changing too many variables at one time to be able to figure out what is causing your itching.

When things came to a head, and I stopped my injections, my Hematocrit was a 53.9. The Hematologist said it was high, but not dangerous.

The carrier oil was something that I was suspect about but I have been through 2 other brands and my 6 weeks being off TRT.

I like your theory of the iron deficiency. I have been reading more and more about this - Can I just pop into a GNC and start taking this?

The too many variables isn't true. When I started my itching several months ago I tried topicals, less showers, anti-histamines and whatnot, all while on TRT (i switched the brand at the onset of the symptoms but stayed with that brand for 3-6 more months with the same symptoms.

Last week (after 6 weeks off) I was waiting for my new doc's script to be filled and tried another cyp source but yeah....that was dumb and I should of waited for my new pharma stuff. It was 1 shot though.

I just looked at all my old labs and non ever had iron.....only Vitamin D from other culprits.... that was normal

No, you don’t take iron unless you have verification that your ferritin levels are low. Serum iron tests are unreliable and inaccurate. Ferritin and iron saturation are gold standard. CBC can be normal and you can still be iron deficient.

Looks like I haven’t had a good blood panel since 2023 and at that time my Ferratin was 53. Says it was on the very low end of normal. I’m waiting on results from a full panel that I have went in for last week.
 
How is your liver, itching can be too much bilirubin under the skin

It appears that I got off of my Pharma test at some point a year ago and wound up getting my test from a local source and that is what my issue started happening

After some discussion today with my doctor, we determined that my original testosterone source was from a compounding pharmacy and they use different preservatives. I believe that it’s possible that I might be allergic to something in the mass produced test causing my allergic reactions.

Post labs FT, hemoglobin, hematocrit and RBCs.

We always want to test at the true trough (lowest point) before your next injection which would be 7 days post-injection seeing as you are injecting once weekly!

If you tested at the true trough and you are still hitting a very high TT 1000-1300 ng/dL than you are clearly overmedicated!

Even then although TT is important to know FT is what truly matters as it is the active unbound fraction of T responsible for the positive effects.

If you truly are hitting an absurd trough TT 1000-1300 ng/dL then your trough FT would be high/sky-high and the reason for your elevated hematocrit/RBCs!

Better yet your peak TT/FT would be ridiculous.

Sounds like you are dealing with one of those dime a dozen run of the mill T-clinics that jack men up on T from the get-go!

Luckily they never threw in the AI (devil's candy) which is a common theme here!

Most men on TTh are injecting 100-200 mgT/week whether once weekly or split into more frequent injections as in twice-weekly (every 3.5 days),M/W/F, EOD or daily.

Most men can easily hit a healthy let alone high or in some cases absurdly high trough FT injecting 100-150 mg T especially when split into more frequent injections.

Yes there are some outliers who may need the higher-end dose but it is far from common as in RARE!

200 mg/week T would be overkill for the majority!

Running a high let alone absurdly high trough FT is a surefire way to drive up your H/H and RBCs which leads many men to get caught up on that donating too frequently merry go round in order to manage such.

It is pretty much a given that you will eventually crash your ferritin which can open up another can of worms!

If you are one of those outliers or what we call hyper responders then you may need to pay attention to the carrier oils/excipients of the particular formulation.

Even then some may even have a bad reaction to the ester!




I ran cyp from this compounding pharmacy for almost 3 years with no issue - then I stopped seeing this doctor....got cyp from a local source for the past year but also gave blood and tried other things and when I retraced all my steps I think it all started when I stopped taking this particular test from this particular compunding pharmacy.

When I stated to post labs for FT, hemoglobin,hematocrit and RBCs it was for your TTh protocol 200 mg T/week as you never stated this was your summertime routine let alone your standard dose being 100 mg T split twice-weekly (50 mg every 3.5 days).

We always test at the true trough (lowest point) before the next injection as we need to make sure FT level is not too low or too high as the goal is improvement of low T symptoms while at the same time keeping overall blood markers healthy let alone minimizing/preventing any sides!

Again although TT is important to know FT is what truly matters as it is the unbound fraction of T responsible for the positive effects.

Critical to know where your trough FT let alone other important blood markers H/H and RBCs sit on said protocol (dose of T/injection frequency).

Any doctor in the know would want to know where your trough FT sits!

Where did your trough FT and RBCs, hemoglobin and hematocrit sit on your standard dose 100 mg T split twice-weekly?

Where did your SHBG sit pre/post TTh?

Also critical to keep an eye on iron/ferritin if you are one who gets caught up on the donating too frequently merry go round trying to manage elevated hematocrit due to running a high/absurdly high trough FT!
