In the past three decades, several pharmacological options have improved the treatment of erectile dysfunction (ED). Each treatment has unique efficacy and safety profiles. Tailoring oral therapy with PDE5 inhibitors (PDE5is) based on patient profiles, such as age, relationship status, and intercourse frequency, is crucial. Oral PDE5is can be daily or on-demand, depending on patient needs. If oral treatments fail, or if there are contraindications, intracavernous injection (ICI) therapy with alprostadil can be considered. Combining multiple treatments in parallel has become a common approach. Regenerative therapies like shockwaves and platelet-rich plasma (PRP) show promise, but further research is needed. PDE5is will likely remain the most prescribed ED treatment.
