Is Free T4 needed when testing Thyroid?

I had a quick look at Nelsons check list for blood work while on HRT and noticed it says just to test TSH and Free T3. T4 being optional.

Can anyone explain the need or not to test T4?

Yes, It is important to know how T4 is converting to T3 and RT3.
