High Total T and Very low Free T

I am trying to figure out what to do. I have high Total T, LH, FSH, and Estradiol, but very low 0.45 ng/lL Free T. I just got my blood drawn for SHBG to confirm that the SHBG levels are high.

Is there an AI that will do a good job lowering SHBG? I read about something called AIFM (ATD), but have not been able to find any information about it online.

Testosterone, Serum 902 Range: 264-916 ng/dL 01
Free Testosterone(Direct) 4.5 Low pg/mL Range: 6.8 - 21.5 02
LH 15.1 HIGH Range: 1.7-8.6 mIU/mL 01
FSH 11.7 NORMAL Range: 1.5-12.4 mIU/mL 01
Estradiol 36.9 NORMAL Range: 7.6-42.6 pg/mL
Prostate Specific Ag, Serum 0.4 ng/mL 0.0 - 4.0 01

Thanks!!!

Indeed, an AI won't do much for you. Your high LH is suggesting that your testosterone production is maxed-out. Even if your estradiol result were confirmed via LC/MS, and even if all of it was prevented, this would raise total testosterone by only about 3.7 ng/dL. (Though reduced estradiol might help lower SHBG some). As @Systemlord says, you need TRT, or even TRT plus danazol.

SHBG is likely skyhigh and as noted you can't directly manipulate SHBG in one direction or the other. I wouldn't think any one would touch you for TRT as the standard for HypoGonadal is much lower, 250(?) in TT. You might find a Dr that will treat you based on Free T but technically might not be able to make a medical diagnosis possible.
