Here are the FDA-approved label indications for the anabolic steroids nandrolone, oxandrolone, stanozolol, and oxymetholone:
Nandrolone
- FDA-Approved Indications:
- Management of anemia of chronic kidney disease (renal insufficiency) by increasing hemoglobin and red cell mass12.
- NOTE: Nandrolone at 150 mg biweekly was found to be more effective in increasing lean body mass than high dose growth hormone (6mg or 18IUs/day) in a HIV study.
Oxandrolone
- FDA-Approved Indications:
- Adjunctive therapy to promote weight gain following extensive surgery, chronic infections, or severe trauma.
- Management of protein catabolism associated with prolonged administration of corticosteroids.
- Relief of bone pain associated with osteoporosis3.
- NOTE: Oxandrolone 20 mg/day plus TRT was effective in increasing LBM with milder LFT’s elevations.
Stanozolol
- FDA-Approved Indications:
- Treatment of hereditary angioedema to reduce the frequency and severity of attacks5.
Oxymetholone
- FDA-Approved Indications:
- Treatment of anemias caused by deficient red cell production, including:
- Acquired aplastic anemia.
- Congenital aplastic anemia.
- Myelofibrosis.
- Hypoplastic anemias due to myelotoxic drugs or chemotherapy6911.
- NOTE: Oxymetholone was effective in treating HIV wasting but caused significant increases in liver enzymes.
- Treatment of anemias caused by deficient red cell production, including:
Nandrolone and oxandrolone, both anabolic steroids, have several off-label uses in addition to their approved medical indications. Below is a summary of their off-label applications:
Off-Label Uses of Nandrolone
- HIV/AIDS-Associated Wasting: Used to preserve lean body mass in patients with wasting syndromes, such as those associated with HIV/AIDS13.
- Burn Recovery: Administered to promote recovery and support lean muscle mass in patients recovering from severe burns1113.
- Chronic Catabolic States: Utilized for conditions characterized by negative nitrogen balance, such as prolonged illness or trauma13.
- Post-Surgical Recovery: Occasionally used to enhance recovery and counteract weight loss after surgery or severe injuries13.
- Geriatric Weakness: Previously employed to treat general weakness and fatigue in elderly patients, though this use has declined over time13.
- Raynaud’s Phenomenon: In some cases, it is used to reduce the frequency and severity of Raynaud’s episodes6.
- Androgen Replacement Therapy (ART): Proposed for androgen replacement in men and postmenopausal women due to its favorable anabolic-to-androgenic ratio13.
Off-Label Uses of Oxandrolone
- Severe Burns: Widely used to accelerate wound healing, improve lean body mass, and reduce hospital stays in burn patients3814.
- Chronic Non-Healing Wounds: Applied in cases of chronic catabolic states or spinal cord injuries to promote wound healing14.
- HIV-Associated Wasting Syndrome: Prescribed to combat weight loss and muscle wasting in HIV-infected individuals12.
- Turner Syndrome: Used (in combination with growth hormone) to promote growth in girls with Turner syndrome, although this has largely been replaced by other therapies312.
- Idiopathic Short Stature: Previously used to accelerate growth in children with short stature, though its efficacy for increasing adult height is limited312.
- Chronic Obstructive Pulmonary Disease (COPD): Occasionally used to counteract muscle wasting associated with COPD12.
- Alcoholic Hepatitis: Investigated for improving nutritional status and reducing catabolism in patients with alcoholic hepatitis512.
