Not sure if anyone will respond I know this thread is 4 years old..
But still a big topic for my life...
I'm 39 and I've been on trt for about 7 years...
I didn't start HCG till the 2nd or 3rd year when my testicles became completely atrophied...

Cut to present day I have developed BPH, awful... And I'm attributing a % of it to the testosterone therapy over the years because I am still relatively young and this started 2 years ago when I was 37.....

I now have testicles the size of marbles and so many lower urinary tract issues. I've been lowering my testosterone on my own and managing my HCG on my own with just my primary doctor and blood work every 6 months..PSA is good
LH and FSH are like the lowest number on the chart... Testosterones around 700 to 850 I have labs if anyone wants I can repost.

I'm down to 65 mg of testosterone cyp. And I'm only taking 200 IU 2x weekly of HCG. I'm wondering if I should update HCG dosage.

My goal is to get off of the trt as I'm aging with this BPH I'm worried that it will make things worse..

And I'm wondering if there's any hope that my testicles will ever get back to even half the size they were with HCG being a higher dose

sorry this got a little off topic..

Anyone that can provide any insight .. and knows of any doctors in North New Jersey that take horizon NJ health.

There is a possible intermediate step before dropping TRT altogether: You could run a trial for a few months with a testosterone nasal gel, such as Natesto. Over time this will allow your HPTA to recover. It will provide robust daily peak testosterone, while lowering average levels, which might be helpful.

A couple of ideas for you...if TRT is involved in your BPH it is probably via DHT so you might get that checked, HOWEVER it is much more likely that your BPH is due to some sort of infection, especially given your UTI issues. DR. Gundry recently talked about this on episode 299 IIRC of his podcast where he said that there is increasing evidence that BPH is often caused by a bacterial infection that is driven but intestinal permeability (AKA leaky gut) , and he said that he reveresed his own BPH by getting his leaky gut resolved. I have also heard of Doctors treating BPH with antibiotics, which again is consistent with the infection model. You may be best served by finding a good holistic doc who understands infection issues to get a good workup and anti-infection protocol. In the meantime You could investigate a totally clean diet such as that recommended in the book Perfect Health Diet by Paul Jaminet which is one of the steps to fixing leaky gut, although that alone may not resolve an infection. Liposomal Vitamin C and several grams of Vitamin C a day, and getting your Vitamin D levels over 50 are also must-do's as a bare minimum to start with.

Also, if you are in an otherwise good place with TRT, it seems reasonable to focus on the infection side of the equation first for BPH. You can try more HCG for testicle size, and I have found that clomid (and probably enclomiphene) also preserves testicle size even though many people claim it won't.
