bad shot

Hey all I have done a deltoid shot today, shortly after the muscle has started to twitch/spasm(4 hours ago), just wondering the cause behind this as I have never had it happen in a few year's - Currently using 29g x 12.7mm needles I usually also tend to push the needle in a little harder. Also...
