A simple therapeutic trial of thyroid medication will tell you pretty quickly if there's room for optimization.
With that elevated of a TSH I personally think it's worth a shot if you've got a good/open-minded doc.

I've gone back and forth using Lugol's drops and the actual tablets/pills.

I don't think you'll want to be on either long term, but to get the adrenals functioning properly so you can get back to optimizing thyroid and testosterone.

I think around 6 weeks or so I started slowly adding back in the iodine and thyroid NDT.
I get much better absorption I've noticed when I supplement the NDT with iodine.

I think your situation was a lot like mine.
Seems most guys have cortisol that's too high from stress or whatever.
I was always on the low end, so getting adrenals working properly before getting thyroid dialed in was 10x more trickier for me than most.

So it looks like 200mg of Test a week, 75 IU's of FSH EOD.
If he's going through a 10,000 IU bottle of hCG in two weeks with EOD shots, that sounds about or aound 1500 IU's of hCG EOD which is a considerable amount as well.

Having RT3 would help, but regardless I agree with everyone above.....you're going to need to treat thyroid.
And just for the heck of it.........when you're saying iodine is optimal, can you be specific as to what you mean?

This has to be hampering your goals.
I'd pursue treatment for sure.
Probably low dose NDT with T3 or T3 exclusively.
Would be worth your while to examine the remainder of your hormone profile as well at this point.
